NDIS Physiotherapy Services
Offered from our locations in Joondalup, West Perth and Osborne Park
We are now offering NDIS Physiotherapy services for Plan and Self Managed NDIS clients.
Our primary focus is on children aged 7+ to adults with disabilities and can be provided from any of our locations in Osborne Park, West Perth and Joondalup. However, our Joondalup location is best suited for children and teens if you are near that area!
What is the NDIS?
The NDIS stands for the National Disability Insurance Scheme and is designed to provide funding to people with disabilities to help them gain more time with family and friends, greater independence, access to new skills, jobs, or volunteering in their community, and an improved quality of life.

How does it work at Formotion Physio and are we suitable for you?
Formotion Physio operates from within boutique style gyms in Joondalup (Mobius Health & Performance), West Perth (Inner City Fitness) and Osborne Park (Range of Motion).
Each of these gym are a separate business to us, however we work alongside each of them either with their clients or our own, and with access to the facilities they provide.
As Physiotherapy providers this extra space and equipment allows us to provide the more physical therapies to improve:
Mobility and physical function
Self care and self management techniques
Pain management related to your disability
We also absolutely love this setup, as it allows for much more of a community feel than a typical Physiotherapy clinic. You may find this is equally good for you, or your loved one who is on the NDIS, as it allows us to provide the essential elements the NDIS aims to provide like:
Ability to build confidence and social skills in a community environment
Social interaction in a real world environment
During our therapies we aim to incorporate all these elements to help you reach your specific goals.
Assessments and Reporting
We are also able to provide assessments that may assist in NDIS applications as well as NDIS reports and recommendations to assist you in accessing the funding you need.
Fee Policy:
Our fees range from between $200 - $220 / hour depending on the therapist you see and the requirements of your sessions with us. These will be discussed after your first session when creating our service agreement.
This fee includes extra short/ad hoc communication and liaison with other health professionals as a part of the session. i.e short phone calls and letters.
If the time we spend outside of your session becomes extensive we will discuss the need to bill for the time required.
